Meniere´s Disease treatment: Pressure Pulse Therapy


The pressure pulse therapy is a relatively new (not yet very widespread or simply been ignored) form of therapy, although initial studies exist on this subject for over 15 years (Densert B, Densert O, Arlinger S et al. Am J Otol 1997; 18: 726–733).
Pressure pulse therapy was derived from experiences with Meniere's patients who were treated in a hyperbaric chamber.
It could be observed that the symptoms of Meniere's disease can be affected by pressure changes (Densert O, Ingelstedt S, Ivarsson A et al. Acta Otolaryngol 1975; 80: 93–100).

With pressure pulse therapy, low-amplitude pressure pulses with a maximum thickness of 20-25 cm water column will usually be applied to the middle ear by a before inserted ventilation tube (Gates GA, Green JD. Laryngoscope 2002; 112: 1489–1493; Franz B, van der Laan F. Int Tinnitus J 2005; 11: 146–149).

The exact mechanism of action is not fully understood. It is assumed that applied pressure pulses counteract the endolymphatic hydrops, possibly through a promotion of endolymph effluent or interference of endolymph production through reflex mechanisms (Densert B, Densert O, Arlinger S et al. Am J Otol 1997; 18: 726–733; Gates GA, Green JD, Tucci DL et al. Arch Otolaryngol Head Neck Surg 2004; 130: 718–725).

The pressure pulse therapy is used for seizure prophylaxis, but is also applicable during a seizure.
Patients treated by the therapy are in a position to intervene actively in the disease, which has a positive effect on the psyche of patients. Thus may also contribute to improve quality of patient´s life conditions.
Numerous studies have been demonstrated that the pressure pulse therapy is an effective method for Meniere's disease treatment.

Pressure pulse therapy should be applied by those sufferers in which conservative and non-destructive therapies (e.g. drug therapy) have not shown a satisfactory effect.

In summary, it must be said that the pressure pulse therapy should not be overstated as a panacea to cure Meniere´s Disease, but the pressure pulse therapy should not be ignored as an alternative and non-invasive treatment option, and it should definitely be offered to sufferers before an invasive, destructive therapy, such as gentamicin instillation or a surgical procedure.
